Old Colony Elder Services Awarded Grant

I am pleased to share the news that Old Colony Elder Services, Inc. in partnership with Beth Israel Deaconess Plymouth has been awarded a grant of up to $300,000 under the state’s Hospital to Home Partnership Program. The purpose of the grant is to improve discharge processes from the hospital and ease capacity constraints while supporting patients in returning to their communities.

It is part of a larger grant cycle of $3 million awarded by the Healey-Driscoll Administration to acute care hospitals and Aging Services Access Points (ASAPs) in Massachusetts. This program aims to improve the discharge process from hospitals and support patients in returning to their communities.


The grants will be used to build partnerships between hospitals and ASAPs to strengthen communication and coordination with community providers. Hospital and ASAP partners will work together to ensure that the appropriate services and support are in place for patients to transition directly to home and community-based settings after discharge, with appropriate services and support.
